WORKSHOP REGISTRATION FORM

or call (212) 966-6823.  You will be asked to email the completed registration form below to hold a place in the class.

To register please send a $200 deposit check made payable to Brian Bomeisler together with the registration form below.  All classes in NYC take place at 51 Greene  Street in Soho, Lower Manhattan.  The balance of your tuition is due 21 days prior to the start date.   Personal check or direct bank wire transfer are acceptable.

**Complete the form and mail with your $200 deposit check for each workshop or full tuition.  Checks made payable to Brian Bomeisler and sent to:

** $200 deposit is non refundable within 21 days of the workshop start date.  A $50 cancellation/handling fee will be applied prior to 21 days.  Tuition balances are due 21 days prior to workshop start date.  There are no refunds after the full tuition has been paid.
